We have been proudly serving the citizens of Caddo, Bossier, Webster, Claiborne, Bienville, Red River, and Desoto Parishes for over 25 years. We aid and assist individuals with disabilities and the elderly with a holistic and centered approach to meet their personal care needs.

How To Apply For Services
We will assist you with enrolling into Long Term Care. Once your application has been completed Long Term Care will contact you and
complete a medical assessment with you in your home.
After the evaluation with the state assessor, a representative from Long Term Care will send you an eligibility letter in the mail informing you of your status for approval or denial. Once approved contact our office for further assistance.
